Please include:

1. Your company name - or the text you would like to go in its place (see example below for type style, effect, color and size)

2. Your name and any text to go underneath Your company name. (Your Name MBA in the example below)

3. Your email address for the bottom of the page (a clickable link)

4. Your website address for the bottom of the page as well. Also a clickable link.

5. How many templates you would like. (see costing below)

6. Which template background patterns you would like. (see chart at bottom of page)

To have all 25 templates personalized in stationery pack six the cost is $75.00

To select 10 templates and have them personalized the cost is $65.00

To select 5 templates and have them personalized the cost is $50.00

To select 1 template and have it personalized is $30.00.
